- Make certain all tubes or ear notches are clearly labeled & sealed.
- Place tubes or ear notches in air-tight plastic freezer zip-lock bag(s).
- Place bag containing tubes or ear notches into shipping box surrounded with ample packing material to prevent tube breakage.
- Place completed submission form and payment into shipping box.
- Use shipping labels provided and any cautionary labels required on outside of box.
- Choose method of shipment to ensure delivery to lab on Mondays or Tuesdays.
